2. Complete "Qualified Individual" check out
   • Schedule with Brian Angel
   • Takes roughly 2 hours in classroom and on the water
   • Price break when taken with another person
   • Q.I. Status lasts 2 years
3. Sign "Qualified Individual" Waiver form
   • Must be updated yearly

2. Reserve Dates for Rental
   • Minimum 4 days advance notice required
   • Complete online rental request
   • No cancellations permited. No Refunds.
3. Collect and file "Particpant waiver forms" for all passengers on board
   • A parent or Guardian must sign for children under age 18
   • Leave in black Mailbox across from KHYC office
4. Leave check for rental fees in KHYF mailbox
   • Located across from KHYC office
